Introducing the Trans Health Card, with EMPOWER
Trans Health Card: A tool to facilitate affirming conversations between gender-diverse patients and their health care providers.
Trans Health Card: How to use: 1. Download and print the Trans Health Card, or ... 2. Pick up a blank Card at ACT or other partnering locations. 3. Input the information you wish to share with your care providers. 4. Fold the card like an accordion. 5. Present your Trans Health Card along with your Ontario Health Card when accessing healthcare services.

Because they were trans or non-binary, 12% had avoided going to the emergency room in the past year, despite needing care." Trans Health Card: Refer to a service user: 1. If a service user indicates they are trans, non-binary, or gender non-conforming, offer to them a Trans Health Card. 2. Introduce the Trans Health Card as a tool to help communicate key identity markers and gender-specific health information to service providers and administrators. 3. Encourage them to fill in the information they feel comfortable sharing with others. 4. Instruct them to present the Trans Health Card whenever they use their Ontario Health Card.
